Oak City Pickleball Registration Form - Spring 2025

We've designed this league to be a doubles-oriented experience; if you have a doubles partner registering with you, that is perfect (please name each other in the appropriate spot on the registration form and we'll keep you together); if you need a doubles partner, we will do our best to pair you with someone who matches your skill set – there is not any sort of guarantee with this! We're a work in progress but we are settling into a groove (Spring 2025 will be our second year and fourth season of operation), so please come in with that level of understanding, and be prepared to meet people and have fun!

Spring 2025 Individual Player Registration Fee

Oak City Pickleball League Spring 2025 Season Individual Player fee is $80.00. Please note that registration is NOT COMPLETE until your fee is submitted. This fee is NON-REFUNDABLE. By registering and paying your player fee, you are acknowledging that you understand your fee is non-refundable. You also acknowledge that games played on fields owned by the owner of our rented facility are subject to that entity's decision regarding facility playability due to any condition.

Spring 2025 Season: April 4, 2025 – May 12, 2025.
12 matches per doubles teams are planned for six Mondays at our rented facility; all games are governed by facility declarations mentioned in the above paragraph.

I authorize Oak City Pickleball League and Oak City Athletics to use any image captured on film or video for publication on its website or other league-related promotional purposes without further consideration.

Code of Conduct
Before registration is complete, we would like to communicate to all potential players that we have a code of conduct that we expect everyone to agree to and submit as part of becoming a player in our league. We are interested in building a supportive, respectful program that is a safe place for everyone who is a member. In that regard, please read, understand, and agree to the following code of conduct:

1) There will be zero tolerance for verbal or physical abuse of any kind, including threatening behavior, toward players or staff (this includes Oak City Pickleball League leadership team members and/or facility staff, security, or associated persons). This behavior will result in an immediate and permanent ban from Oak City Pickleball League.

2) There is no scenario where stealing of equipment or any other item belonging to another party is acceptable.

3) There is no scenario where intentional defacement or damage of Oak City Pickleball League's facility or its environment is acceptable.

Any of the above three items, when reported or noticed by league officials, will be the subject of a meeting of officials and immediate scrutiny, which may result in a reprimand and a warning. A flagrant violation may include a permanent ban from league participation; this is at the discretion of league officials after reviewing the scope and severity of actions being analyzed. We take the safety of our league members seriously, and we are determined to promote a respectful environment.

Player Liability Waiver

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. 

2. I knowingly and willingly assume the risks of playing the game, both known and unknown, even if arising from the negligence of others, and assume full responsibility for my participation. 

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. 

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Oak City Athletics, Oak City Pickleball League, and their directors, officers, officials, agents, volunteers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
